I looked at the photos in the OP where the woman was able to flip the implant around while inside her buttocks, and it kinda raised an anatomical question: why doesn't that shit hurt?

Imagine somebody made a large incision in your asscheek and pulled that shit open. That would hurt. Now imagine he put his finger into the gaping wound and started poking around in there. That would hurt even worse, because you'd have a foreign body (the finger) wreaking havoc inside the tissue and stimulating nerve firing and thus pain.

Yet here you have a woman with a huge foreign object inside her ass shifting it around and in so doing displacing her natural ass tissue and disturbing the nerves. Why doesn't that hurt?

You don't have those nerve endings on the inside. The pain of cutting your skin or poking a finger in the wound are the result of damage of the surface. Nerve endings have evolved at places where pain is expected so you recognize the danger, and more important places have more nerves. That's why paper cuts in your hands are so painful: you really need your hands.

During open brain surgery they remove part of your skull and stimulate ('poke') your brain tissue while the patient is conscious. The patient can then tell the doctors whatever they experience, e.g. a memory, a smell or colors. The patient doesn't feel the touching itself because there's no nerve endings for that inside the brain.
