Doctors, nurses, and scientists aren't essential for running a country - the bureaucrats will always have their doctors, and firing some "unruly" police officers is actually a plus for them as they only need enough police to SWAT dissenters and crush protests, they don't actually need to maintain law and order, especially when the disorder is not directed at them.
Regarding China, this anarcho-tyranny really only works in the West for a few reasons. First off, the Chinese people, like most East Asians, are less violent, more orderly, and more compliant by nature, so anarcho-tyranny would be more difficult to implement. White westerners are slightly less compliant, but they don't have the in-group preference asians do, so the natural orderliness of white western societies can more easily be "corrected" by bringing in third world migrants, many of whom are not compliant people.
So the ruling class has determined that for China, classical tyranny works best, but for Western nations, anarcho-tyranny works best in maintaining control.
