Yeah, most American guys, especially those over 30, don't have a clue when it comes to sizing. I was with some coworkers getting a company shirt, and they all requested sizes one to two sizes too big.
Haha. Now that's one of the most accurate observations on here. For guys it's all because of very misplaced vanity. If a guy is a "small" he will insist he is "medium", If he is a "medium" he will insist he is a "large" etc.
At one company outing I was almost the only guy who's polo actually fitted properly since I was likely the only guy who wrote down "S" on a form a few weeks earlier. You know, since I actually AM a fairly small guy. Every other guy wrote down something at east one size to big. Fine when you're writing it on a form, but you look like a clown when you actually have to wear something much too big for you.
But to actually comment on the OP:
To be honest a female doctor is not going to be happy with what she considers trading down. In her case she ideally needs to look for someone like a somewhat older and established medical specialist to balance the forces of nature.
The problem for her, and all women really, is that what women consider trading down, and what men consider trading down are worlds apart. A professional woman will be looking for a more qualified man (at least for a LTR). But that more qualified man will be using his value to attract a hot woman, not necessarily professional woman.
A woman with a position high on the totem pole (like a doctor) is pretty much screwed when it comes to LTR's. Unless she is also hot, but this is extremely unlikely. She has narrowed her pool of prospective men to the size of a puddle, while really not doing anything to increase her own attractiveness (Though of cause she imagines her qualifications have done so).
