For the basic $125 claim you don't need to submit any documents. To claim more money I believe you need to submit proof to prove that you froze your credit and/or were negatively affected by the breach.

They only reserved 31 million for payments, most people wont get anything. That doesnt even cover 1% of the people affected.

The equifax breach had effected us in the UK are we eligible to file a claim? I was using clear score that was a subsidiary of theirs. They since sold that to someone else.

It asks for your name and last 6 social security digits.
It will then say if you were affected, then you choose to either get a $125 prepaid card, or if you want more, submit documentation.
