You hit the nail on the head with how you know if a person or company is buying fake followers though: engagement. So if they have a huge number of followers yet get fuck all for likes on individual posts, then you have your answer. I have an ex that "only" had 2500ish followers that was getting upwards of 10% engagement per post (solid numbers) and I know of many girls that have way over 10k that are getting sub 1% engagement, think 10k followers and less than 100 likes a post.
